Lassen HEARTS @ Public Health

Lassen H.E.A.R.T. (Health Education and Resource Tool) is all about awareness, identification, connection, better access, and collaboration among service providers of all fields to families and children from pregnancy to age 21. Lassen H.E.A.R.T. works to identify children who are at risk of health, developmental, and/or behavioral and emotional problems, and getting them the help they need. It is a program to connect all services and service providers throughout the county and surrounding areas, as well as link families to these services or the information they need to ensure their children’s healthy development. Lassen H.E.A.R.T. encourages, supports, and connects families to help their children grow, learn, and develop to their fullest potential. ASQ-3 developmental screening & ASQ-SE behavioral screenings for children ages 1 month to 5 and a half years are also provided.
