Golden Umbrella/ Multipurpose Senior Service Program

Services: Multipurpose Senior Services Program/Golden Umbrella: These programs provide multi-service referrals, assistance, and counseling to assist seniors and disabled persons to prevent or delay nursing facility placement and remain in their homes, by provides a caring and qualified case manager who will meet with clients and develop a plan that will draw on existing community resources and services. The primary objective is to avoid or delay the inappropriate placement of persons in nursing facilities, while fostering independent living in the community. Services may include adult day care, housing assistance, minor home repairs and maintenance, non-medical home equipment, emergency move, restoration of utility services and temporary lodging. Nursing assessment is a vital part of the MSSP program and eligibility is addressed through screening process and program participation.
Hours: 8:00am – 5:00pm, Mon-Fri
Fees: None
Eligibility Requirements: 65 or older, receiving Medi-Cal and living in service area, and certifiable for placement in nursing facility.
